Contractors and companies use this report to track safety performance and submit it to clients at the end of each month.

Whether you’re managing a construction site, industrial plant, or any high-risk workplace, a properly structured Monthly HSE Statistics Report helps maintain transparency, promote safety awareness, and improve compliance with safety regulations.

What is a Monthly Safety Report?

A Monthly Safety Report includes all relevant safety statistics, training activities, safety violations, and audit recommendations. It helps clients and contractors evaluate the effectiveness of safety practices over the reporting period.

Monthly Safety Report Format – Five Key Parts

You can prepare a comprehensive Monthly HSE Report by dividing it into the following five sections:

1. SAFETY STATISTICS

This section contains important HSE numbers such as:

  • Average Manpower = Total manpower in the month ÷ No. of days
  • e.g., 900/30 = 30
  • Total Manhours Worked = Total manpower × Working hours
  • e.g., 900 × 9 = 8100 hours
  • Lost Time Accidents (LTA) – If an injured worker cannot resume work within 48 hours, it counts as an LTA.
  • Man Days Lost
  • Medical Treatment Cases
  • First Aid Cases
  • Near-Miss Incidents
  • Fire Incidents
  • Safe Manhours – Total working hours without LTA
  • Frequency Rate = (No. of LTAs × 1,000,000) ÷ Total manhours
  • Severity Rate = (Man days lost × 1,000,000) ÷ Total manhours

2. TRAINING / AWARENESS PROGRAM

Record the safety training and awareness initiatives:

  • No. of staff and workers trained in safety induction
  • No. of Toolbox Talks
  • No. of HSE awareness programs

3. INSPECTIONS / AUDITS

Track monthly safety inspections:

  • Site safety inspection
  • PPE (Personal Protective Equipment) inspection
  • Store inspection
  • Other HSE inspections

4. SAFETY VIOLATIONS

Log the number and source of violations:

  • Violations raised by contractor
  • Violations raised by client

 SAFETY AUDIT RECOMMENDATIONS

Include recommendations from any internal or external HSE audits conducted during the month.
